Please complete the Hurricane / Emergency Pet Release Form to provide emergency contact information and acknowledge Bocage Animal Hospital’s emergency closure policy.

In the event of a hurricane, tropical storm, or other declared emergency, Bocage Animal Hospital may be required to close for the safety of our staff and clients. During an emergency closure, our hospital cannot house or shelter pets, and no staff will remain on-site to provide care or monitoring.

  • All pets must be picked up before the hospital closes, whenever possible
  • No staff will remain on-site to provide care or monitoring during the emergency
  • The hospital will be inaccessible until it is deemed safe to reopen

Owner Acknowledgment

I understand that Bocage Animal Hospital cannot provide boarding or sheltering services during an emergency. I am responsible for arranging the safe pickup and care of my pet(s) prior to closure. I authorize the emergency contacts listed above to retrieve my pet(s) if I am unable to do so.
